This item was opened but never used. We bought this (new from Target) as an extra to keep at my mother's house. She opened it, installed batteries and set it up in the nursery at her home (with the tags still on it, see photo 3). Unfortunately, our son outgrew it before he ever had a chance to use it at her house. We removed the batteries, placed it back in the original package and stored away.
All the original toys and components are with it and also unused.
There are a few small dust marks on the underside corners where it was moved around by her housekeeper. It is underneath the mat so a child using this would not have contact with that area. The mat can be removed and washed to remove the dust marks. Since we never used it, I'm going to leave that to the buyer's discretion.
This is from a smoke-free, pet-free home.
For safety reasons, I have verified that there are absolutely no recall issues with this item. :)
